Programs for unsheltered and precariously housed people in Northern Sonoma County. Street Outreach, Housing, and Homelessness Prevention.
The Reach for Home Program is based on three main pillars: Street Medicine and Outreach, Housing and Eviction Diversion. The goal of our programs is for our clients to create a path to find long-term housing, generate income, and build functional life skills.
Street Medicine and Outreach
Our Outreach Team builds foundational trust with our clients by visiting them daily. We provide basic food and supplies, transportation to essential appointments and case management in the field including meeting each individual where they are in their journey to housing. We also offer a community health and wellness mobile unit to bring services to the streets including an on staff Community Health Worker (Promatora).
Housing
We provide housing units for individuals and families who are the most vulnerable in our community. We work with a team of providers to empower our participants to build the skills necessary to maintain stable housing. Reach for Home case managers work with participants to develop financial know-how, build resumes, practice effective parenting, and advance other essential life skills.
Homelessness Prevention
We work with several partners and funding sources to help those who are on the edge of losing housing to ensure they can stay housed. This includes rental and utility assistance. We work to build relationships with landlords to identify those who would benefit from our program. Our goal is to keep individuals and families from becoming homeless.
The Reach for Home staff teams with clients to meet important milestones on the road to stable housing. Case Managers create personal action plans, provide living spaces, and teach families and individuals financials skills. While working on their Skills for Life, clients are able to increase income levels so that by the time they reach the end of their stay, they can attain successful housing outcomes.
